This online application must be completed by the agency principal who will be electronically signing all documents. To begin our new online application process, please register your information below.

Additionally, to expedite completion of initial phase of information gathering, you will be asked to upload the following docs. Please have these documents ready to access and upload:

  1. Agency’s Texas General Lines License(s),
  2. Agency Errors & Omissions Declarations Page or Certificate of Insurance (including any Additional Named Insured or Additional Location Endorsement),
  3. If Agency uses an assumed name or operates additional locations, submit acknowledgement from TDI of registration of the assumed name or additional location,
